The Community of Constituents initiative is a statewide movement of nearly 1,000 organizations, with more than 300 affiliated members, working together to transform the system of care so that all Californians can age with dignity, choice, and independence. This initiative includes the Sacramento-based California Collaborative for Long Term Services and Supports (CCLTSS) and 20 county-level Regional Coalitions.
